Please remember:
- All reimbursements above the maximum we determine for the child will be the responsibility of the parent
- Parent and childcare provider must each sign attendance sheets, which are considered legal documents, and may NOT be signed for by one another.
- The choice of childcare provider is determined by the PARENT, not by any individual in the program.
- Submit monthly Attendance Logs at the end of each service month
